Who this covers
This website, mypaindoc.org, is run by MYPAINDOC, P.C. (“MYPAINDOC,” “we” and “us” below). This policy explains what the website collects, why, and the choices you have. It covers this website only.
If you become a patient, your medical information is protected by our separate Notice of Privacy Practices and by the federal and state laws that apply to health care, not by this website policy. Records of addiction treatment are also protected by federal confidentiality rules (42 CFR Part 2), as that notice explains. You can ask us for a copy at any time.
Is your Notice of Privacy Practices posted online? If so, send us the link and we’ll add it here. Please don’t send health details through the website
The contact form and email aren’t the place for symptoms, diagnoses, medications or other health details. Share only what we need to get back to you, and we’ll talk about the rest privately by phone or in person. In an emergency, call 911.
What the website collects
- The appointment request form: your name, phone number, preferred office, the kind of care you’re asking about and anything else you add. It’s saved in the website’s own database and passed to our staff so they can call you to schedule.
- Emails, calls and texts: if you contact us directly, we receive what you send, along with your contact details.
- Visit statistics: which pages are viewed, and the website that sent you here (just its name, not the full address). A random visit code, kept by your browser only until you close the tab, groups one visit’s pages together. These statistics don’t include your name or use cookies.
- Technical information: like every website, the host receives your device’s internet (IP) address and browser type when you load a page, to deliver it and protect against attacks.
How we use it
To reply to you and provide what you ask for, to keep the website working, secure and easy to use, to block spam, and to keep reasonable business records.
We don’t sell your personal information or share it for targeted advertising.
Services that help run the website
These providers handle information only to provide their service:
- Cloudflare: hosts the website and protects it from attacks.
- Dana Wright Co. (DanaWright.co): builds and manages the website. Form messages are stored in its client dashboard and passed on to us, and its notification and reply emails are sent through Resend. The visit statistics described above are kept there too.
Cookies
The website itself doesn’t use cookies to identify you. You can block or delete cookies in your browser settings; the website still works without them.
How long it’s kept
Messages and contact details are kept only as long as we need them to reply, to do the work you ask for, and to keep reasonable business records. You can ask us to delete them at any time. Do you have a set schedule for deleting old website messages (for example, after two years)? If so, we’ll state it here.
Visit statistics are kept to compare traffic over time; they don’t identify you.
Your choices and rights
You can ask what information we hold about you, ask us to correct or delete it, or ask us to stop using it. Depending on where you live, you may have further rights under your state’s privacy law. We’ll respond within 30 days.
Keeping it safe
The website uses an encrypted (HTTPS) connection, and access to messages is limited to the people who need them, including the people who run the website for us. No internet service is perfectly secure, so please don’t send sensitive information such as Social Security, bank or card numbers through the website. That includes health details.
Children
The website isn’t meant for children under 13, and we don’t knowingly collect their information. If you think a child has sent us information, please let us know and we’ll delete it.
Changes to this policy
If this policy changes, the new version will be posted here with a new date.
